چکیده
Peroral endoscopic myotomy (POEM) is a form of transluminal natural orifice surgery (NOTES) that uses the beginnings submucosal endoscopy to make equivalent surgical myotomy. POEM can be done in most patients who have primary idiopathic achalasia demonstrated manometrically symptomatic. Previous therapies for achalasia, such as pneumatic balloon dilation, botulinum toxin injection, or myotomy, are not contraindications POEM. Generally, 4 continuous steps: incision mucosa, construction tunnel, and closure mucosa. Endoscopic management has been revolutionary treatment other pathological entities, however, it must performed specialized centres by endoscopist surgeons with extensive experience subject, reduce associated morbidity mortality.

Achalasia is a motility disorder of the esophagus, characterized by aperistalsis of the esophageal body and incomplete relaxation of the lower esophageal sphincter (LES). Treatment of achalasia is currently aimed at decreasing the resting pressure in the LES. Peroral endoscopic myotomy (POEM) is an emerging novel endoscopic procedure for the treatment of achalasia with initial data suggesting a...

Peroral endoscopic myotomy (POEM) has been developed in the context of natural orifice transluminal endoscopic surgery (NOTES) as a minimally invasive endoscopic treatment for symptomatic esophageal achalasia, which is a chronic progressive benign disease with severe morbidity and difficult management. Since September 2008, POEM has been successfully performed in more than 200 consecutive patie...
متن کاملPerspective on Peroral Endoscopic Myotomy for Achalasia: Zhongshan Experience
Worldwide, peroral endoscopic myotomy (POEM) has achieved remarkable initial outcomes in the treatment of achalasia. In China, POEM has developed very quickly since the first case was performed in our center in August 2010. With ex-perience, we have successfully performed POEM for special cases (such as pediatric patients, patients with sigmoid-type esophagus, and patients with recurrent sympto...

A 54-year-old woman sought clinical consultation because of a > 27-year history of dysphagia. She had an Eckardt score of 13 and was diagnosed with sigmoid-type achalasia (S2 type, type II) based on esophageal motility testing, upper endoscopy, barium esophagography, and clinical presentations. We investigated a technique of peroral endoscopic myotomy (POEM) performed directly without a submuco...

Peroral endoscopic myotomy (POEM) is the application of esophageal myotomy to the concept of natural orifice transluminal surgery (NOTES) by utilizing a submucosal tunneling method. Since the first case of POEM was performed for treating achalasia in Japan in 2008, this procedure is being more widely used by many skillful endosopists all over the world. Currently, POEM is a spotlighted, emergin...
